1. Present ID and fill up request form at the LCR Office If requesting party is not the document owner, present written authorization from the document owner

Marriage Certificate
• LCR Copy: Php 200.00 /copy
• PSA (NSO) Copy: Php 265.00 /copy

CENOMAR 
• LCR Copy: Php    50.00 /copy
•PSA (NSO) Copy: Php 320.00 /copy

2.  For PSA (NSO) Copy request is submitted to the PSA Serbilis Center every Friday. Copy will be issued the next following working day.
