MT. MALASIMBU

Mt. Malasimbo is a sacred mountain for the locals. A conical-shaped mountain, it is believed that when dark clouds cover the summit of this mountain, a typhoon is coming.

REQUEST FOR ENDORSEMENT OF REGISTERED DOCUMENTS TO THE PSA (NSO)

1. For newly registered certificate
•Inform the LCR to submit the certificate in advance
 
Pay the following fees:
Service Fee: Php 100.00
Certified Copy: Birth/Death: Php   50.00
Marriage: Php 200.00
 
• LCR shall prepare the endorsement letter to nearest or preferred PSA(NSO) outlet Document owner shall shoulder the expenses for courier used.
• Waiting time for issuance of PSA (NSO) copy is 15-30 days
 
2. With PSA (NSO) negative certification received
• Present the PSA (NSO) certificate of no record to the LCR
• LCR shall verify in the archive if the certificate was registered
• If registered, follow the procedure in NO. 1
• If not registered, apply for late registration and follow the procedure for that purpose
