Registration of Birth

Timely Registration : within 30 days from time of birth
Place of Registration : at the LCR Office of the city/municipality where birth occurred
Person Responsible: attendant at birth (hilot, midwife, doctor), administrator of hospital or clinic, Parents of child
Procedure:
1. Submit 4 copies of Birth Certificate (Mun. Form 102) signed by attendant at birth (doctor,or midwife, or hilot ) and informant;
2. If the child’s parents are not married, the Admission of Paternity at the back of the birth certificate must be signed by the father before any administering officer (notary); while the Affidavit to Use the Surname of the child’s Father (AUSF) must be signed by the mother for registration at the LCR Office ; Registration fee for AUSF - P150
3. Copy of the registered birth certificate will be issued within 20 minutes.
Late Registration: (after 30 days )
Procedure:
Submit the following documents:
• PSA (NSO) certificate of no record of birth
• Two (2) or more documentary evidence such as:
Baptismal certificate, Permanent School Record,Voter’s Record, SSS/GSIS/Philhealth Member Data record, Income tax return, birth certificate of children (if applicable), marriage certificate (if applicable), etc.
• Joint Affidavit of two (2) disinterested persons who might have known/witness the birth of the child
• PSA (NSO) certificate of no record of birth
• Two (2) or more documentary evidence such as:
Baptismal certificate, Permanent School Record,Voter’s Record, SSS/GSIS/Philhealth Member Data record, Income tax return, birth certificate of children (if applicable), marriage certificate (if applicable), etc.
• Joint Affidavit of two (2) disinterested persons who might have known/witness the birth of the child

3. If child was born on March 19, 2004 to present, and the parents are not married, the Admission of Paternity at the back of the birth certificate must be signed by the father before any administering officer (notary); the Affidavit to Use the Surname of the Father (AUSF) must also be signed by the mother for registration at the LCR Office (RA 9255);
4. If child was born on August 3, 1988 to March 18, 2004, and the parents are not married, the Admission of Paternity at the back of the birth certificate must be signed by the father before any administering officer (notary); the child shall be under the surname of the mother (Family Code);
5. If child was born before August 3, 1988, and the parents are not married, the Affidavit of Acknowledgment at the back of the birth certificate must be signed by the father and mother before any administering officer (notary); the child shall use the surname of the father (Civil Code);
6. Copy of the registered birth certificate will be issued after 10 days during office working day.
