BFP-FIRE SAFETY INSPECTION SYSTEM (FSIS)

An online platform that allows users to apply for Fire Safety Evaluation Clearance (FSEC) in Building Permit and Fire Safety Inspection Certificate (FSIC) in Business Permit for New and Renewal, and in Occupancy Permit. The FSIS is designed to streamline the application process and expedite the issuance of certificates and clearances.

STEP 1: REGISTER

*Create your account, visit fsis.e-bfp.com

* Fill-up the necessary information and click "SIGN-UP"

*Check your "INBOX" or "SPAM" on your email for account validation

STEP 2: APPLY

*Once registered, "LOG-IN" your account. Click "APPLY NOW" on your dashboard and select the application you need

a. Fire Safety Evaluation Clearance (FSEC)

b. Fire Safety Inspection Certificate (FSIC) for Occupancy or Business Permits, and

c. Clearances

* Upload the necessary requirements to complete your application

STEP 3: PAYMENT

* After checking the completeness of your application, the ASSESSOR will assess necessary Fire Code Fees.

* You can "VIEW" or "PRINT" the order of payment for you application

*Pay the assessed fire code fees, click "PROCEED TO PAYMENT" to process your payment

STEP 4: EVALUATION INSPECTION

*Designated Fire Safety Evaluator or Inspector will "EVALUATE" or "INSPECT" based on application submitted.

*Check the recommendations or notices in your account dashboard

STEP 5: RELEASING

*You can now "VIEW" or "DOWNLOAD" your certificate or clearances by clicking "NEW DETAIL" on your application.

Online Payment Procedure


STEP 1

• Full Name of Lot Owner
• Location of Property
• Copy of previous receipt (optional)

Note: Wait for our reply containing the “Order of Payment”

STEP 2

ONLINE PAYMENT, click the link provided in the email and follow the procedure for payment

PAYMENT LINK: https://www.lbp-eservices.com/egps/portal/index.jsp

- Click PAY NOW
- Enter Merchant Name, search for “dinalupihan” and choose MUNICIPAL GOVT. OF DINALUPIHAN and hit continue
- Select “Real Property Tax” in the transaction
- Fill-up required fields

     Amount
     Tax Declaration Number
     Contact Number
     Name of Taxpayer
     Email Address
     Payment Mode

Note:
1. Screenshot payment if successful and send to This email address is being protected from spambots. You need JavaScript enabled to view it. (for verification). Once verified, official receipt will be printed.
2. For OFW’s or clients residing outside DInalupihan, your Official Receipts will be kept in the Treasurer’s Vault for safe keeping and can be claimed anytime upon presenting the screenshot of your payment

Registration of Birth

Timely Registration :      within 30 days from time of birth
Place of Registration :    at the LCR Office of the city/municipality where birth occurred
Person Responsible:      attendant at birth (hilot, midwife, doctor), administrator of hospital or clinic, Parents of child

Timely Registration :     within 10 days  after the court order became final
Place of Registration:    at the LCR Office of the city/municipality where the court function
Person Responsible:    clerk of court; petitioner

Timely Registration :     report within 48 hours  to  the Municipal Health Officer and register within 30 days  from the time of death
Place of Registration:    at the LCR Office of the city/municipality where event occurred
Person Responsible:     attendant at death, or administrator of hospital or clinic, or nearest relative
