Application of Marriage License
Procedure:
1. Applicants must submit the following documents:
Filing of Petition for correction of clerical error in the Registered Birth, Marriage o Death Certificate (Republic Act 9048 & Republic Act 10172)
Place of Petition: at the LCR Office of the city/municipality where the certificate was registered
Person Responsible: Document owner, parents, children, brother or sister, or any person authorized the document owner
Registration of Birth
Timely Registration : within 30 days from time of birth
Place of Registration : at the LCR Office of the city/municipality where birth occurred
Person Responsible: attendant at birth (hilot, midwife, doctor), administrator of hospital or clinic, Parents of child
Registration of Court Order
Timely Registration : within 10 days after the court order became final
Place of Registration: at the LCR Office of the city/municipality where the court function
Person Responsible: clerk of court; petitioner
Person Responsible: clerk of court; petitioner
Registration of Death
Timely Registration : report within 48 hours to the Municipal Health Officer and register within 30 days from the time of death
Place of Registration: at the LCR Office of the city/municipality where event occurred
Person Responsible: attendant at death, or administrator of hospital or clinic, or nearest relative

Registration of Legal Instrument
Timely Registration : within 20 days after the Legal Instrument is executed
Place of Registration: at the LCR Office of the city/municipality where the certificate is registered
Person Responsible: Affiant/interested party to the Legal Instrument
Person Responsible: Affiant/interested party to the Legal Instrument
Registration of Marriage
Timely Registration : 15 days after marriage (with marriage license) 30 days after marriage (exempted from marriage license)
Place of Registration: at the LCR Office of the city/municipality where event occurred
Person Responsible: Solemnizing officer
